Depression Screening: Implementation of a Validated Screening Tool in Rural Primary Care

Depression and anxiety are two common mental health disorders in the U. S. Despite recommendations for routine screening using validated tools by the United States Preventive Services Task Force (USPSTF) and the American Academy of Family Physician (AAFP), regular depression screening is not occurring in primary care. The purpose of this project was to integrate USPSTF recommendations for depression screening, using the PHQ-2 and PHQ-9, to increase screening at all annual and follow-up visits in adult patients, aged 18 and older, of one provider practicing in a rural primary care clinic in southeast Kansas. The AIMS of this project were to (1) increase the number of adult patients screened for depression using a validated tool in the participating provider’s practice and (2) assess provider satisfaction and perceived impact of validated screening tool integration. Formative evaluation included descriptive evaluation of workflow processes. Summative evaluation included partial assessment of the number of screenings performed. The provider had a total of 20 days in clinic with 260 total recorded patients screened. The provider journaled throughout implementation and provided insight through a semi-structured interview upon completion. She found an overall ease of use and reported increased depression recognition and diagnoses using the tool. Barriers reported were occasional patient reluctance. Timely recognition of depression and proper treatment plans drove positive patient outcomes. The provider expressed value in implementation clinic-wide. This project supported feasibility of implementation and use of validated screening tools in rural primary care to help providers recognize depression early and promote prompt treatment.
