Abstract
Advance Care Planning (ACP) is a crucial aspect of healthcare decision-making, yet it often overlooks young adults who are considered healthy and less prone to contemplating mortality. This thesis addresses this gap by highlighting the importance of including young adults in ACP initiatives. This thesis proposes strategies to integrate ACP into their transition from parental health insurance to individual coverage. Recognizing that young adulthood is a period of significant life changes and transitions, including turning 26 and aging out of parental health insurance, this research explores leveraging this key life event to engage young adults in ACP discussions.This innovative approach seeks to seamlessly incorporate ACP discussions within the context of young adults' health insurance decision-making. Thus, making it a natural part of their transition to independent health coverage. By coupling ACP with health insurance enrollment, this strategy aims to increase awareness, facilitate discussions, and encourage the completion and annual review of ACP decisions.In addition to this integration, the thesis proposes the incorporation of a digital tool to facilitate ACP discussions and documentation. This tool could provide personalized planning based on user comfort level, educational resources, and prompts for ACP conversations, making the process more accessible and engaging for young adults. By leveraging technology, the aim is to overcome barriers such as lack of awareness and facilitate ongoing ACP engagement.
