Implementing a Diversion Assessment Protocol for a Detention Diversion Program

Abstract Problem Statement: Adults with behavioral health disorders are over-represented in the criminal justice system in Jackson County, Missouri. Detention can negatively impact behavioral health symptoms, access to appropriate treatment, individual productivity as a citizen, and budgetary constraints of the justice and health systems. Purpose and Aims: The purpose of this quality improvement project was to implement a diversion assessment protocol and determine appropriateness of justice involved adults, age 18 to 75, who have behavioral health needs, to proceed in a detention diversion program. The project was implemented in Jackson County through a Department of Mental Health forensic mobile team. Aims included evaluation of risk, functional status, community tenure, and program engagement. Background: Nearly two thirds of individuals with repetitive justice system encounters in Jackson County, have a behavioral health disorder. As many as 40% of those with serious mental illness and 50% with substance use disorders go without treatment in this community. Research has demonstrated that evaluation of risk and clinical status can support diversion as a measure to reduce criminal justice contacts and promote recovery of individuals with behavioral health needs. Methods: Kotter’s model for change was operationalized to organize team members, and the model for improvement was utilized to implement processes. Tools used for data collection included the HCR-20V3, the WHODAS 2.0 clinician and self-report versions, an active recovery measure tool, and a clinician survey. Data was analyzed using a paired sample t test to compare outcomes pre- and post-intervention. Descriptive statistics were also examined.
