Identifying the Obstacles to Breast Cancer Screening in a Suburban Primary Care Practice: A Population Barriers Assessment

Abstract
Background: Screening mammography is a proven method of secondary prevention of breast cancer. Patient adherence to screening recommendations often requires education, reminders, ease of access to screening methods, and addressing fears and perceived barriers. Effective strategies are necessary to maximize the completion rate and reach the highest potential benefit of breast cancer screening. Purpose: This project aimed to evaluate barriers to the completion of breast cancer screening within a suburban environment with the intent of increasing overall breast cancer screening at a primary care clinic. Method: A population barriers assessment and process analysis were used for this quality improvement project. A primary care population barriers assessment survey was completed by 27 women meeting breast cancer screening criteria within a suburban community. Patient attitudes about breast cancer screening practices, clinic practices and procedures, and community resources were analyzed. Descriptive statistics were used to communicate to the clinic leadership for future intervention development. Conclusion: The pooled results of the population barriers assessment, community assessment, and process analysis showed multiple areas for improvement: 1) Improve clinic processes for scheduling of the mammogram, 2) Improve the clinic procedure for follow-up on patient given an order for a mammogram, 3) Improve the patient utilization of the patient portal, 4) Use of the electronic medical record to improve proactive patient reminders and 5) Use of patient-directed breast cancer awareness educational interventions.
