Mental Health Workflow Integration in a Midwest Gastroenterology Clinic for Adults

Abstract
Problem: Failure to meet best mental health practice recommendations leads to high costs and poor patient outcomes. Previous efforts to increase mental health resources in a Midwest academic health center GI clinic failed due to the lack of concrete clinic mental health data and cost-effective solutions. With mental health needs expanding, a systematic, sustainable approach was needed to improve mental healthcare access, quality, and cost.Aim: The purpose of this hybrid QI needs assessment was to: a) standardize the mental health screening and referral process for adult GI patients, b) document the prevalence of mental health needs, and c) assess the need for integrated mental health services into the GI clinic.Method: Lewin’s change theory was used to create and implement the GI mental health workflow. The project spanned 26 weeks and included five phases: 1) workflow creation, 2) integration, 3) RN education and training, 4) workflow implementation and data collection with two-week PDSA cycles, and 5) data analysis to assess the need for integrating mental health services in the GI clinic.Results: The GI clinic mental health workflow was accurately utilized 10 times. Six of 10 patients screened positive. Four of six patients agreed to a mental health referral.Limitations: Utilization of the workflow was less than anticipated. The project sample size was small.Conclusion: A mental health workflow in the GI clinic was generally accepted and effectively identified patients needing mental health services. Prevalence did not support the need for integrated mental health services. Conclusions must be interpreted with caution.
