Evaluation of the Interdisciplinary Management of Uncontrolled Type 2 Diabetes Mellitus in the Primary Care Setting

Abstract
Problem: Type 2 diabetes mellitus (T2DM) affects 425 million people worldwide and is associated with high healthcare utilization and cost. Effective management of T2DM is complicated by gaps in care due to multiple factors, including poor care coordination and poor patient adherence. A large midwestern academic medical center identified a gap in interdisciplinary communication and care coordination for patients with uncontrolled T2DM that is associated with poorer outcomes. To improve chronic care management, interdisciplinary communication, and outcomes for patients with T2DM, an interdisciplinary diabetes team was developed.Aim: The purpose of this project was to evaluate the effect of an interdisciplinary approach to diabetes management in the outpatient setting for patients with uncontrolled T2DM at a large midwestern academic medical center.Method: This quality improvement project was a part of an initiative at a large midwestern academic medical center to identify patients with T2DM with an A1c greater than 9.0 and a history of high utilization of services. During this pilot, patients were referred to a team of pharmacists and chronic care managers for interdisciplinary management and referral to other disciplines. Patient and referral data were collected through chart reviews.Results: There was a 24% reduction in hospitalizations, a 38% reduction in the total number of ED visits, and an average of a 22% reduction in A1c. Limitations were a small sample size, poor patient retention, and minimal referral to all disciplines in the IDT.
