Implementation of I-PASS Handoff Communication Tool to Improve Transitions of Care

Abstract
Problem: The Joint Commission ranks communication failures among the most common attributable causes of sentinel events in healthcare – with gaps in handoff communication posing significant risk. A standardized transition of care handoff communication tool is pertinent for improvement in the transition of care handoff communication by nurses.Aim: The purpose of this project was to standardize nursing handoff communication in transitions of care, decrease adverse patient events due to ineffective handoff communication, and promote positive patient outcomes and experiences by implementing the I-PASS transition of care handoff communication tool.Method: This quality improvement (QI) project utilized a phased approach that included the Lewin Change Model and project management to implement the I-PASS transition of care handoff communication tool as the primary mechanism for handoff communication in the hospital setting.Results: Need to implement a standardized handoff communication tool was established. Barriers to handoff communication were identified by nursing staff. I-PASS tool templates were implemented, trialed, and modified through PDSA cycles and feedback provided by nursing staff.Conclusion: Future evaluative findings could support the I-PASS handoff communication tool as a mechanism to decrease adverse events related to communication. While multiple limitations challenged project completion, implementation of the I-PASS handoff tool offers insight to the role of complexity science in QI projects – and the adoption, implementation, and sustainability of moving evidence into practice.
