Reducing 30-day Readmission Rates in Post-Operative Cardiac Surgery Patients

Abstract
Problem: Heart disease continues to be the leading cause of death in the United States. Cardiovascular surgery, including coronary artery bypass grafting and/or vavular repair, may be required as heart disease progresses. Due to the increased risk of adverse outcomes and increased financial burden on healthcare facilities, it is important to identify methods to reduce 30-day readmissions in post-operative cardiac surgery patients.Purpose: Reduce project facility 30-day readmission rates to be at or below Society of Thoracic Surgeons-like group readmission rates through implementation of an advanced practice provider telephone follow-up, utilizing a standardized script, within three days of discharge in adult patients who have undergone cardiac surgery. The target performance benchmark for 30-day readmission rates for coronary artery bypass grafting was 9.85%, for aortic valve replacement was 9.90%, and for combination coronary artery bypass grafting with aortic valve replacement was 11.55%.Methods: This quality improvement project occurred over a five month period. Data analysis included cardiac surgical procedures, patient demographics, count and percentage of follow-up telephone calls within three days of discharge, use of standardized script, and patient readmission within 30-days of discharge.Results: There were zero 30-day readmissions after implementation of the follow-up telephone call putting facility 30-day readmission rate below Society of Thoracic Surgeons-like groups.Limitations: Limitations included small sample size, patient discharged location, and limited evaluation of 30-day readmissions to outside facilities.Conclusion: Utilizing a patient-centered approach for individualized follow-up after discharge can decrease 30-day readmission rates, financial burdens on the healthcare system, and improve overall patient outcomes.
